This lecture explores the diverse range of intraoral donor sites that can be used for autologous tissue harvesting beyond what is typically covered in dental school and training. By utilizing autologous tissue, we can avoid the risk of disease transmission and take advantage of its growth factors. However, the gold standard tissue is often overlooked due to concerns around donor site morbidity. Selecting donor sites that are adjacent to the recipient area is associated with minimal postoperative discomfort. Specifically, the focus is on the utilization of tuberosity bone, translocated ridge segments, and extracted or partially extracted teeth. Additionally, complications that may arise when biological principles are not followed will be reviewed. This lecture’s goal is to expand the knowledge of intraoral donor sites and provide practical guidance for their successful utilization in clinical practice.
Snježana Pohl is an oral surgeon and periodontist with dual doctorates in human and dental medicine. She is an oral surgeon in Rident, a private clinic in Croatia. In addition to her clinical practice, she holds a faculty appointment as an Assistant Professor at the University of Rijeka’s School of Dental Medicine, Department of Oral Medicine and Periodontology. Snježana Pohl is the author of numerous scientific papers. Her work focuses on the importance of minimally invasive techniques that preserve tissues. She has particular expertise in Partial Extraction Therapies, Autogenous Tissue Utilization, Osseodensification, and other innovative approaches to implant surgery. Advanced Immediate implantation and immediate loading in the aesthetic zone Systematic planning prepares you for a case requiring) an immediate implant with immediate loading. The main advantages of immediate implant placement are that they save time and there are fewer patient visits. During implant surgery, the dentist has to make a lot of decisions that are based on scientific knowledge and his own experience and personal skills. There are numerous indications for tooth replacement with an immediate implant when an adequate amount of bone and soft tissue are available to support it. Teeth need to be removed atraumatically to preserve the maximum amount of bone before immediate implant placement. Placement of immediate implants is a predictable procedure and attention to detail is essential to ensure success when placing these implants. There is the possibility to improve the Pink esthetic zone with immediate hard and soft tissue augmentation. The harvesting of CTG from the palate for soft tissue management can improve the result in the aesthetic zone. This lecture will include hints and tips on all aspects of immediate implantation, hard and soft tissue management with CTGs, which you can take back to your practice. He is a Specialist in Oral Surgery and Implantology and has an additional degree as Master of science in Periodontology and Implant therapy from the German association of Periodontology. He also received his Master of Science in Esthetic Dentistry (Goethe University, Germany) Goethe University of Frankfurt has granted him the Habilitation and venia legendi for dental medicine. He received the Award for Excellence in Teaching three times from Goethe University, Frankfurt ( 2012, 2017, 2023). Currently, Puria Parvini is working at the department of oral surgery and implantology at Goethe University in Frankfurt, where he is is the Chief Senior Physician and Absentee’s Representative of Clinic Director Department of Oral Surgery. Dr. Puria Parvini has special focus in the field of complex bone augmentation, dental trauma, soft tissue management around implants and teeth, plastic-aesthetic periodontal surgery, management of complications in oral surgery, emergencies in dentistry. He has more than 14 years of experience with immediate implantation and immediate restoration. Dealing with Esthetic Failures on Implants in the Esthetic Zone – From a Soft Tissue Management Perspective
The common causes of esthetic failures around implants in the smile zone will be presented, such as implant malpositioning and inadequate soft tissue support, and various approaches for managing these issues will be described in detail throughout several clinical examples.
Soft tissue management, such as soft tissue grafting techniques, type of connective tissue and their clinical behavior will be discussed in detail along with their indications and limitations. Comprehensive treatment plan that takes into consideration the patients’ esthetic concerns and clinical factors that could prevent esthetic failures will be addressed, as well as the need for ongoing maintance and monitoring, to ensure the long-term success of the final complex implant and prosthetic restoration.

Simplified lateral ridge augmentation by f.i.r.s.t. and one-time cortical lamina
The Fibrin Sealant (FS) is a drug of human origin used in general and specialist surgery for many decades. It is validated by over 6,000 papers in the literature. In oral surgery, FS stabilizes and maintains the natural clot for 10-14 days. It has adhesive, sealing and inductive action for hard and soft tissues. FS allows better management of particulated bone graft that, becoming plastic and adhesive, adapts perfectly to defects without shedding of granules. It also protects surgical wounds and stimulates faster soft tissue healing. The F.I.R.S.T., devised by Foti V. in 2016 and published in 2020, is a GBR technique that stands out for its simplicity, predictability and low morbidity. Based on the triad fibrin sealantcollagenated heterologous bone-heterologous cortical lamina, it allows the reconstruction of two/three walls post-extraction sockets using envelope flaps (Lamina Shield). In lateral GBR the F.I.R.S.T. allows to glue cortical laminae for heterologous bone particles containment without pins or screws. The result is a lamina/graft complex strongly adherent to the defects, without micromovements and empty spaces, that is one of most important prerequisites for the neo-angiogenesis and the new bone formation. Finally, the minimally invasive “One-Time Cortical Lamina” technique, published by Foti V. in 2021, where it is achieved a vestibular lateral ridge augmentation to simultaneous implants that does not require surgical re-entry for uncovering.

Osteo-Immunology : A New Paradigm Shift in Implantology and Bone Grafting
Osteoimmunology is the modern science that shows the influence of immune system on bone metabolism but also on all tissue healing phenomena. Indeed, bone healing starts with an immune reaction: inflammation triggered by the release of inflammatory cytokines. These cytokines are responsible for angiogenesis, which activates the formation of bone callus. The behavior of bone cells is dependent on the functioning of the immune system. High level production of oxidants is named oxidative stress, will lead to a failure of the immune system with collateral effects such as infection, bone resorption or fibro-integration. This is the case, for example, in diabetics, smokers, patients allergic to penicillin or in chronic inflammatory diseases. A favorable bone metabolism will necessarily require a reduction of oxidative stress by means of various solutions such as adequate vitamin D supplementation, reduction of LDL cholesterol levels or other adequate immune stimulants. Antibiotic prophylaxis must also be revisited in order to use antibiotics with significant immune-stimulating properties. PRF has also an immune function : decreases the inflammatory level and increases the local anti- oxidants. This new science favored new clinical protocols : the open wound technique. With a powerful immune system there is no fear of infection, It will be easy to demonstrate that peri-implantitis is an immune disease that causes bone loss around the implant, triggered by oxidative bone stress.

When approaching an extreme dental patient, usually involving severe atrophy of the bone available for implant rehabilitation, we should be equipped with a variety of solutions. Advanced technologies enable us a relatively safe and predictable way to treat almost any case, making procedures simplified, treatment time shortened, and offering our patients a significant improvement in life quality regarding function, aesthetics, health, and long-term maintenance. Several methods will be presented independently or integrated followed by a relevant literature discussion, complications and ways to deal with them, materials available, and long-term follow-ups.

Decoding the aesthetic zone in implant therapy: When, what and how for optimum aesthetics
Once the decision is made to extract a tooth, clinicians have a decision to make; delayed management – i.e. extract and wait, a staged approach – i.e. graft and wait or immediate implant placement. Immediate implant placement combined with a later or an immediate soft tissue support? Also, the achievement of an aesthetic implant-supported restoration is a constant challenge to the restorative dentist. Due to the circular shape of the implant and its smaller diameter, when compared to the root of a natural tooth, a dilemma inevitably occurs of how to construct an artificial crown that will imitate the natural tooth crown form. The appearance of the surrounding soft tissue the is of major importance, and various techniques have been developed to guide its topography. This presentation will highlight parameters and key factors for the evidence-based decision making in order to choose the technique that facilitates a more aesthetic, functional and predictable result according to the patient expectations. Starting with diagnostic models through surgery with immediate provisional restoration therapy to the definitive restorations.
Dr. Konstantinos D. Valavanis attended the National and Kapodistrian University of Athens Dental School, Greece, where he received his degree in dentistry in 2000. After graduating he was trained in implant dentistry in Kraichtal Germany by Prof. Ady Palti and the Goldman School of Dental Medicine, Boston MA. He was then trained in prosthetics by Prof. Nitzan Bichacho, Tel Aviv, Israel. Dr. Valavanis is a senior partner at the Athens Dental Institute, Athens, Greece and Adjunct Professor at the Oral Surgery Department, University of Naples Federico II, Italy Dr. Valavanis currently serves as the President of ICOI-Hellas, Ambassador and Member on the Board of Directors of the International Congress of Oral Implantologists (ICOI), Board Member of the SENAME Implantology Association (Naples-Italy), Board Member of the Athens Dental Association and a fellow of the International College of Dentists (ICD). He is the originator of the Smart Casual Dentistry Symposium, founding member and implantology specialist of the German Implantology Society (DGOI), Dental XP expert and Diplomate in the International Congress of Oral Implantologists (ICOI). Today many approaches have been described for alveolar bone regeneration. In our opinion a case 3D analysis is mandatory as well as a prosthetic and aestetich implant installation. To obtain stable results during the follow-up, bone and soft tissue should be managed in accordance with a precise prosthetic plan. We will expose clinical and scientific evidences of the Computer Guided Bone Regeneration approach.
Prof. Silvio Mario Meloni DDS, PhD, MS Associate Professor , University of Sassari, Italy Graduated in Dentistry with honors and mention of merit in 2003 at the University of Sassari, in 2008 he obtained a PhD in Preventive Odontostomatology and in 2009 the Master of Science in Oral Surgery at the same University. In 2012 he completed the gIDE / UCLA One-year Master Clinician Program In Implant Dentistry, at UCLA University, Los Angeles where he was awarded with the Award: 1st Place Top Honors in Implant Dentistry. From 2010 to 2017 he was adjunct professor and assistant professor of Periodontology and Implantology. From 2018-2021 he was University Researcher RTDB Assistant Professor of the aforementioned courses for the degree course in Dentistry at the University of Sassari. From December 2021 he became Associated Professor at the School of Dentistry, University of Sassari. In the 2013 he obtained, as the first Italian together with prof. Marco Tallarico, the EAO Certificate for fundamental skills and knowledge in implant based therapy, awarded by the Board of the European Association for Osseointegration. In 2019 he won the Aesculapius Research Prize, awarded by the University of Florence at the VII Aesculapius Colloquium. He is international speaker on issues related to implant surgery, and author of several research projects and scientific publications with high impact factor. Formely active member of the Italian Society of Osseointegrated Implantology, (SIO), active member of the Italian Academy of Osseointegration (IAO), formerly Active Member of European Association for Osseointegration (EAO Certified), active member of the Computer Aided Academy (CAI Academy). He was also the creator of the innovative Computer Guided Bone Regeneration method. He was an Associate Professor in the Department of Periodontics at the University of Southern California. He is a Diplomat of the American Academy of Periodontology, a Diplomate of the International Congress of Oral Implantology with a special Award of 25 years of contribution to The American Board of Periodontology, a Member of Honor of the American Dental Implant Association and Awarded Top of the World Leading Dentist. Dr. Michael Klein, D.D.S., is a renowned specialist in Oral Implantology and Prosthodontics. He graduated from the University of Maryland Dental School and completed a General Practice Residency at Woodhull Medical Center in Brooklyn, New York. He pursued a two-year full-time Fellowship in Oral Implantology and Biomaterials at Brookdale Hospital Medical Center and specialized in Prosthodontics at the University of Medicine and Dentistry of New Jersey.
A former Clinical Associate Professor at New York University’s Department of Periodontics and Implant Dentistry, Dr. Klein is a Diplomate of the American Board of Oral Implantology/Implant Dentistry. He pioneered computer-guided dental implant surgery technology, holding multiple patents, co-authored The Atlas of Oral Implantology, and founded Implant Logic Systems, which he sold to BioHorizons Implant Company in 2008. Currently, he directs Advanced Implant Dentistry and Oral Restoration in New York, maintains a private practice, and serves as Chief Technology Officer at KeyStone Dental Group, where he simplifies full-arch solutions by optimizing workflows.

Prof. Saadoun received his Degree in Dental Surgery from the Faculty of Paris and completed his Post-Graduate Certificate in Periodontology at the University of Pennsylvania and Post-Graduate Certificate in Implantology at the University of California in Los Angeles.
He was an Associate Professor in the Department of Periodontics at the University of Southern California and a Visiting Professor at Loma Linda University, UCLA, the Hadassah Faculty of Dental Medicine of Jerusalem University, and the Faculty of Dental Medicine Mohamed VI.
He is a Diplomat of the American Academy of Periodontology, a Diplomate of the International Congress of Oral Implantology with a special Award of 25 years of contribution to The American Board of Periodontology, a Member of Honor of the American Dental Implant Association and Awarded Top of the World Leading Dentist.
