As a multidisciplinary subject in oral medicine, oral implantology will not only become a standard therapy, but will also be increasingly influenced by general and specialist trends in medicine. As a result, conventional therapy concepts and medical devices will be challenged and/or replaced by disruptive innovations and the latest scientific findings. In particular, minimal invasiveness in osteotomy, new insights into the per-implant soft tissue interface and smart prosthetic therapy concepts fulfill the basal medical goals of a high and sustainable safety of a patient’s integrity. In addition, the reduction of the complexity of individual therapy steps as well as new formats of education contribute to a steadily growing number of clinicians who place implants as well as provide prosthetic restorations.  This is supported by the digital workflow based on a virtual patient using 3D imaging, real-time tracking of hand-guided dental handpieces and the use of artificial intelligence. Furthermore, the new understanding of the disease of peri-implant tissue and consequently innovative therapy concepts promote the clinical long-term prognosis of implants. The aim of the presentation is to demonstrate in clinical cases these already available innovations and to point out the advantages for patients and/or clinicians.

בעידן ה-Free Hand, רפואת השיניים הייתה אומנות שתוצריה תלויים במידה רבה בכישוריו המקצועיים והאישיים של הרופא. כיום, אנחנו עומדים בפתחו של העידן הדיגיטלי, המביא עמו סטנדרטיזציה של תכנון הטיפול, תהליכי העבודה והתוצרים המתקבלים. התעשייה הדנטלית שואפת להגדיל את מספר הרופאים המשתמשים בטכנולוגיות CAD/CAM, ולכן היא חותרת להנגשת הטכנולוגיה לרופא השיניים הסביר – אך תוצריה מוגבלים לחומרים מסוימים הנוחים לעיבוד, והטכנולוגיה אף גורמת לאובדן מיומנויות אישיות בקרב רופאי השיניים והטכנאים כאחד. בנוסף, הסטנדרטיזציה חוטאת גם באובדן הגישה האישית לכל מקרה קליני.

לאורך שנות קיומו של תחום השיקום על-גבי שתלים, נוכחנו לדעת שעבודה ידנית קפדנית, בשיטות אנלוגיות, מובילה לתוצאות טיפוליות טובות יותר מאשר עבודה דיגיטלית, וזאת בשל אי-דיוקים מובנים במערכות הדיגיטליות ובשל הצורך בסטנדרטיזציה. אך יחד עם זאת, בשנים האחרונות השתפרה גם רמת הדיוק של הדיגיטל, וכעת סוף-סוף ניתן לדבר על תחילתו של גישור הפער בכישורים המקצועיים בין רופאי השיניים השונים.

בהרצאה יוצגו גישות שונות בשיקום על-גבי שתלים תוך שימוש באמצעים אנלוגיים (free hand) ודיגיטליים, תוך עמידה על ההבדלים האיכותיים ביניהם. ההרצאה תתנהל כדיאלוג ובשלב מסוים אף כדיון בשיתוף הקהל, ויוצגו בה מקרים קליניים אשר יראו כי אמנם העידן הדיגיטלי כבר כאן, אך גם כבודו של ה-free hand מונח במקומו. יוצג גם האופן שבו שיטות דיגיטליות מתקדמות יכולות לתעד את הידע המקצועי הארטיזנלי הטמון בשיטות העבודה הוותיקות, להפוך אותו לשיטתי ולהנגיש אותו לציבור רחב של רופאי שיניים. 

Rehabilitation of the severely atrophic maxilla presents one of our greatest challenges. Restoring function with efficiency and long term predictability is the goal of doctor and patient alike.The lecture explores historical options and current treatments until these days and shows how the “graft-less” solution withstands the test of time. In my lecture I will introduce the transnasal implant as a viable alternative to the anterior zygomatic implant.

Longevity and esthetic of dental Implants are a real challenge for the clinician. Different etiologies are involved but modification of the alveolar ridge, after Extraction, seems to be the main factor. For a better prognosis, dental implant placement requires the entire restoration of the alveolar crest: An “Environmental Protocol”, in which “Harmony” between Hard and Soft Tissues is mandatory. Like in a Tango dance, in which one of the partners “manages the Rythm and the Tempo” and the second one “makes the Show”, Hard and Soft Tissues behave the same way. The presentation will expose different ways to achieve restoration of these tissues, in which Logistics, Timing and the Dynamics of the procedures are also Key Factors to ensure Harmony Several clinical cases and videos will accompany the presentation.

In daily practice we frequently face complex clinical cases with the ambition of achieving a “restitutio ad integrum” using regenerative-reconstructive treatment of hard and soft tissues.
Among the surgical techniques currently described in the literature to regenerate the alveolar bone, one of the best studied and documented is GBR.
The purpose of the presentation is to evaluate the therapeutic indications of the GBR technique according to the initial clinical atrophic situation and to describe the surgical steps “step by step” of hard and soft tissues to achieve regenerative therapeutic success.
